3. Must list the country you live in. If you live in the UK, please indicate where.


4. Please try and refrain yourself from putting names like 'I luv Himeka!'.


5. You don't need to have a website, but if you do, please make sure you have the code up before you submit your form. Please also make sure the site you submit is suitable for all ages.
